PROVISION OF PORTABLE ULTRASOUND MACHINE COMPLETE WITH ALL STANDARD ACCESSORIES AND CONSUMABLES

Brief Scope of Work The Services shall include but shall not be limited to the following stages:
Stage 1
Reference to clause 1.2 of the General Conditions of Contract, the Contractor by not later than three (3) Months after the Date for Commencement shall complete the supply, installation, testing, commissioning of the Portable Ultrasound Machine and training to be provided to the Company personnel as more particularly described in clause 15 herein. Upon the satisfactory completion of the requirements, the Superintendent shall issue the Completion Certificate for Stage 1. In case, the Contractor completes requirements of Stage 1 prior to the stipulated three (3) Months, the Superintendent shall revise the Contract Period accordingly by issuing a written notification to the Contractor. No Adjustment Order shall be required for such revision to the Contract Period.
Stage 2
Reference to Clause 1.3 of the General Conditions of Contract, the date after the Completion Certificate date for Stage 1 shall be the commencement date of twelve (12) Months Defects Liability Period as more particularly described in clause 16 herein. The Superintendent shall issue Completion Certificate for Stage 2 upon completion by the Contractor of its obligations under the Defects Liability Period.
Stage 3
The Contractor shall carry out the Services in a professional manner to minimize any interference and disruption to the Company’s operations at Ahmadi Hospital and shall take all practical steps which are expected from a professional to prevent harm and keep noise levels to a minimum at all times.
In addition to the details requested under Attachment No. 4 to Form of Tender, if required by the Company, the Contractor shall within a period of seven (7) working Days starting after the date of signing the Contract, submit relevant equipment specification documents, test certificates, drawings to the Superintendent for his approval.
Upon delivery of Portable Ultrasound Machine, the Contractor shall submit to the Superintendent documentary evidence that all the equipment has been received in its entirety without damage or omission of any item and shall produce the Certificate of Origin for the equipment supplied from the manufacturer, stating the Year of manufacture, model number and certifying that the equipment is brand new and not re-conditioned.
Reference to clause 1.26 of the General Conditions of Contract, the date of the Completion Certificate date for Stage 2 shall be the commencement date of the Maintenance Services under the forty eight (48) Months Maintenance period as more particularly described in clause 17 herein. The Final Completion Certificate shall be issued by the Superintendent upon completion of the whole of the Services i.e. at the end of Maintenance period.
The Company reserves its rights to refuse payment for any and all unauthorized Services rendered by the Contractor, and no claims or adjustment orders shall be accepted under the pretext of implied acceptance by the Company, unless the Superintendent had approved such Services in writing
